Requests to Change Instruction Mode for a Course
 
UVA SON faculty members: please complete this form to request approval to change to the instruction mode for a specific course that differs from the instruction mode pre-pandemic.
 
Requests are reviewed by the program director and Associate Dean for Academic Programs. For electives, the program director for the predominate audience will review. This form is intended for graduate courses or courses taken by RN-BSN students.
 
There will be compelling pedagogical reasons to offer some courses in online or hybrid formats. In making decisions to approve a particular course delivery method for a particular course, schools and programs will consider its limits and possibilities. They may consider, for example, how well a particular method achieves goals such as reaching the intended audience; promoting student learning, engagement, and well-being; or broadening access to a diverse range of course content. Once decisions are made, the delivery method for each course should remain consistent throughout the semester barring unforeseen circumstances.
Procedure:
  1. The requesting faculty member discusses the course and the rationale for the proposed instruction mode change with the program director and associate dean for academic programs (ADAP).
  2. The faculty member submits this form.
  3. The admin. assistant to the ADAP forwards to the appropriate program director and the ADAP.
  4. The ADAP consults with the dean and then informs the admin. assistant of the decision for distribution.
  5. The admin. assistant forwards the decision to the requesting faculty member and cc's the SON registrar and department chairs.
