Survey to Assess Congregate Care Telehealth Use, Benefits, and Barriers

The goal of this survey is to assess needs and value of telehealth in your organization. For this survey, telehealth is defined as the use of telecommunications technologies such as phones, computers or tablets to support care between clinicians and patients, professional health-related education, and healthcare administration (such as remote staff meetings). Since 2020, many of us have used (or actively avoided) telehealth as a tool. We want to know about your experience with telehealth and its value for your organization. All survey results will be reported in the aggregate only. This survey should take fewer than 15 minutes to complete.
